present/previous property mgmt co & 12 months cancelled checks if landlord
- Copy of recent appraisal (if 6
months old or newer)

green card (front & back) and
social security card (due to the US Patriot Act)

If divorced:
If self employed:
- Copy of business license
(present and previous years)
- Business card
- Letter from CPA stating they
have done your taxes as self employed for a minimum of two years, or letter
from you stating you do your own tax returns
If you own an investment
property:
- copy of rental agreement
- two years tax returns
If you own a condo or are
purchasing a condo:
- Name of COA, name of property
management company, property manager’s name, address, and phone number
If you are purchasing property:
- Escrow company name and phone
number
- Name and phone number of real
estate agent
